By definition, an _aesthetic_ anus fetishist would care about the appearance of a person's anus. A human anus that resembles the anatomical ideal at rest has symmetrical features; symmetry is one aspect that humans consider when evaluating beauty[2].
Very few people truly appreciate the human anus—aesthetically or otherwise. That means placing a high value upon the good health of such an important body part despite the fact that it is commonly denigrated and abused (intentionally treated in a harmful/injurious manner). If a significant number of people did appreciate it, then rampant, anus-mutilating anorectal abuse would have strong opposition and probably would not be rampant in the first place — nor would it be commonly tolerated.
Partly due to common prejudices plus the aforementioned denigration and abuse, developing an aesthetic anus fetish is highly unusual — and _accepting_ it even moreso. Some malicious people (that is, sexual sadists and sadistic psychopaths) pretend to have such a fetish—and will even praise a clearly damaged/diseased anus typically without being challenged—because doing so facilitates and perpetuates anorectal abuse.
