I can think of two mistakes which he did:
1)Having terrible moderation, jannies were incompetent as fuck
2)At one point, when 4chan shut down due to the sharty hack, he implemented some kind of 24 hour new user timer (which often didn't work) and basically killed the site, at a time when there was a mass exodus of 4chan users. He later stopped it but the damage was done.
